By providing your information in this form and signing a waiver, you are registering to become a member of Cystic Fibrosis Canada’s Elevate program. Elevate is an email-based community input program for people impacted by cystic fibrosis (CF) to help inform CF research, healthcare, support programs and more.
Registrants must:
- Be 18 years or older.
- Reside in Canada.

Your Membership
As an Elevate member, you will receive emails inviting your participation in opportunities including interviews, surveys, advisory committees, meetings and webinars. The opportunities may come from Cystic Fibrosis Canada or the wider CF community, such as universities. Your participation in each opportunity is at your own discretion. You will also receive Elevate impact updates periodically via email that highlight the difference your involvement is having.
Cancelling your membership
You can cancel your membership at any time by emailing the Elevate team at elevate@cysticfibrosis.ca or calling Cystic Fibrosis Canada on 1-800-378-2233.
